v11.12 ships the IVF Outcomes & Live-Birth-Rate program — a single, audited source of truth for pregnancy outcomes that replaces unaudited direct writes, feeds a follow-up worklist with cohort-maturity checkpoints, and powers age-banded, denominator-honest LBR/CPR analytics across both ART and IUI. Alongside it, four legacy paper forms — the ART Report, FET Discharge Summary, Embryo Freezing Report, and Semen Freezing certificate — are rebuilt as structured, versioned digital reports with PDF export, and a clinic-wide pass corrects consent text, timezone, and identity fields across every IVF-generated document. Insights gains a Pulse section for proactive practice signals.
Single audited source of truth for pregnancy outcomes

Eliminates a class of bug where recomputation could null out a real recorded outcome or a previously captured beta hCG value.
Outcome-followup worklist with cohort-maturity checkpoints

▼Live-birth-rate and clinical-pregnancy-rate analytics are computed off the real event stream, banded by patient age, and counted only against the matured cohort (cycles old enough to have a known outcome) rather than every cycle ever started — avoiding the inflated-rate trap of counting too-recent cycles as failures.
Unified LBR view across ART and IUI

Reinstated the dropped "Rapid Progressive" sperm-motility field on the IUI-H report
